Confined Space Versus Permit-Required Confined Space
The first distinction in 29 CFR 1910.146 determines everything that follows. A confined space is large enough and so configured that an employee can bodily enter and perform assigned work, has limited or restricted means of entry or exit, and is not designed for continuous employee occupancy.
A permit-required confined space is a confined space that also has one or more of four characteristics: it contains or has a potential to contain a hazardous atmosphere, it contains a material with the potential for engulfing an entrant, it has an internal configuration such that an entrant could be trapped or asphyxiated by inwardly converging walls or a floor which slopes downward and tapers to a smaller cross-section, or it contains any other recognized serious safety or health hazard.
That fourth characteristic is broad on purpose, and it is where many spaces that employers wanted to classify as non-permit end up.
The employer must evaluate the workplace to determine whether it contains permit spaces, and if it does, inform exposed employees by posting danger signs or by another equally effective means. A space nobody has evaluated does not become a non-permit space by default. It stays unevaluated, which is a different and worse position to be in.
The Four Roles and What Each Owes
The standard assigns duties by role rather than by job title, and training must match the role a person will actually perform.
Authorized entrants must know the hazards they may face including mode, signs or symptoms, and consequences of exposure, properly use equipment, communicate with the attendant as necessary to enable the attendant to monitor entrant status, alert the attendant whenever they recognize a warning sign or symptom or detect a prohibited condition, and exit as quickly as possible when ordered, when they recognize a warning sign or symptom, when a prohibited condition is detected, or when an evacuation alarm is activated.
Attendants have the most demanding continuous duty. They know the hazards, are aware of possible behavioral effects of exposure, continuously maintain an accurate count of entrants, remain outside the space until relieved, communicate with entrants, monitor activities inside and outside, order evacuation under specified conditions, summon rescue, warn unauthorized persons away, and perform non-entry rescues where the plan calls for it. The attendant must not perform duties that might interfere with the primary duty to monitor and protect the entrants.
Entry supervisors know the hazards, verify that the permit is complete and all tests and procedures are done before signing and allowing entry, terminate entry and cancel the permit when required, verify that rescue services are available and the means for summoning them are operable, remove unauthorized individuals, and determine at intervals that acceptable entry conditions are being maintained.
Atmospheric Testing and the Order That Matters
Before an employee enters a permit space, the internal atmosphere must be tested with a calibrated direct-reading instrument for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order.
The order is not arbitrary and it is worth understanding rather than memorizing. Combustible gas sensors typically depend on oxygen to respond correctly, so a low-oxygen atmosphere can produce a misleadingly low combustible reading. Testing oxygen first tells you whether the subsequent readings can be trusted.
Testing must be performed by a qualified person using a calibrated instrument, and entrants or their authorized representatives have the opportunity to observe the pre-entry and any subsequent testing or monitoring.
Continuous or periodic monitoring during entry is required as necessary to determine whether acceptable entry conditions are being maintained. A single pre-entry reading is a snapshot, and conditions in a space change when work begins, particularly when the work itself generates fumes, vapors, or displaces air.
Rescue Is the Requirement Programs Get Wrong
The standard requires the employer to develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue.
Where the employer designates its own rescue team, that team must be provided with and trained to use the necessary PPE and rescue equipment, trained in the assigned rescue duties and in basic first aid and CPR with at least one member holding current certification, and must practice permit space rescues at least once every twelve months using representative spaces.
Where the employer relies on an outside service, the employer must evaluate the service's ability to respond in a timely manner considering the hazards identified, evaluate its proficiency with rescue-related tasks and equipment, select a service that has the capability, and inform the service of the hazards it may confront. Calling the local fire department without confirming they perform confined space rescue, without knowing their response time, and without informing them of your spaces is the single most common failure in this area.
Retrieval systems must also be used whenever an authorized entrant enters a permit space, unless the equipment would increase the overall risk or would not contribute to rescue. The reason this matters so much is well established in incident patterns: a high proportion of confined space fatalities are would-be rescuers who entered without protection to help a downed coworker.
Who Needs This Training
Anyone who will enter a permit space, serve as an attendant, or act as entry supervisor needs role-specific training. Beyond that, employees whose work is near permit spaces benefit from awareness training so that they understand the signs and do not attempt an untrained rescue.
The industries reach further than people expect. Wastewater and municipal utilities, chemical and petrochemical processing, food and beverage production with tanks and silos, pulp and paper, marine and shipyard work, agriculture with grain bins and manure pits, HVAC and mechanical service in ducts and plenums, telecommunications in vaults and manholes, and general manufacturing with pits, hoppers, and process vessels.
Maintenance contractors are a distinct exposure. When an outside employer performs entry in a host employer's permit space, the standard requires specific information exchange in both directions, including the hazards, the entry program, and a debrief on hazards confronted or created during entry.
Employers with spaces they have classified as non-permit should still train the people who evaluated them. A misclassification is a decision, and the people making it need to understand the criteria.
Does Online Training Satisfy the Requirement
The standard requires the employer to provide training so that all employees whose work is regulated by it acquire the understanding, knowledge, and skills necessary for the safe performance of their duties. It does not prescribe a delivery method.
Online training carries the knowledge and role-duty content well. The permit space definition and criteria, the four roles and their specific duties, the permit system, the hazards including oxygen deficiency and enrichment, flammable atmospheres, toxic atmospheres and engulfment, the testing order and why it matters, isolation and lockout, ventilation principles, and the rescue provisions all transfer cleanly.
The skills portion does not. Calibrating and using your specific gas detector, setting up your ventilation on your space, using your retrieval system and tripod, executing your rescue plan, and completing your permit form are hands-on activities on your equipment.
The standard also requires the employer to certify that the training has been accomplished, with the certification containing each employee's name, the signature or initials of the trainer, and the dates of training. Retraining, Documentation, and Practical Guidance
Training must be provided before an employee is first assigned duties, before a change in assigned duties, whenever there is a change in permit space operations that presents a hazard the employee has not been trained for, and whenever the employer has reason to believe there are deviations from the entry procedures or inadequacies in the employee's knowledge or use of those procedures.
Documentation an inspector will request includes the written permit space program, the space inventory and evaluation, completed and canceled entry permits retained for at least one year for the annual program review, the training certifications, atmospheric monitoring records and instrument calibration records, the rescue service evaluation, and the annual rescue practice records where an in-house team is used.
The annual review is a real obligation. The employer must review the program using the canceled permits within one year after each entry and revise the program as necessary to ensure employees are protected. A program with a shelf of canceled permits and no evidence of review is missing a named requirement.
For an employer, the sequence that works is inventory the spaces, classify them honestly, resolve rescue capability before anyone enters, then train to role. For an individual worker, understand that this certificate documents your knowledge of the standard and the role duties, not your authorization to enter any particular space. Entry authorization comes from a permit signed by an entry supervisor for a specific space on a specific day, and the most important thing this training should leave you with is the discipline never to enter a space to help a downed coworker without the equipment and the plan.

How long do we keep entry permits?
Canceled permits are retained for at least one year to support the required annual review of the permit space program. The employer reviews the program using the canceled permits within one year after each entry and revises it as necessary. Keeping the permits without performing that review leaves a named requirement unmet.
