Three things have to be true at once. It is large enough for someone to get in and do work, it has limited or restricted means of entry or exit, and it is not designed for continuous occupancy. A tank, a vault, a silo, a sewer and a pit usually qualify. An ordinary room with a normal door does not, because you can walk in and out of it freely.

A confined space becomes permit-required if it has any one of four things: a hazardous atmosphere, or the potential for one; a material that could engulf someone who entered; an internal shape that could trap or asphyxiate, such as inwardly converging walls or a floor sloping to a narrow section; or any other recognised serious safety or health hazard. Any one is enough. Most spaces people call routine turn out to meet at least one of these once someone actually checks.

A non-permit confined space is one that contains none of those four hazards and no potential for them. It still needs to be evaluated to establish that, and the evaluation is the employer's responsibility rather than a judgement made on the day by whoever is going in. The classification is not permanent either: a non-permit space becomes permit-required the moment the work being done introduces a hazard, and welding or solvent use inside one is the usual way that happens.

The standard defines three roles and they each need their own training. The entrant is the person going in. The attendant stays outside, monitors conditions and never enters to attempt a rescue. The entry supervisor authorises the entry and confirms conditions are acceptable. A team that has trained only its entrants is not compliant, and the attendant role is the one most often skipped, which is unfortunate given how many confined space fatalities are would-be rescuers.

The classroom portion, yes. OSHA does not require a classroom or an in-person instructor for it. What online cannot do is the hands-on part: your own equipment, your own atmospheric testing instruments, your own rescue plan and the specific spaces at your site. That part is the employer's and it happens where the spaces are. Any provider implying an online course alone makes someone a qualified entrant is overselling it.

There is no fixed annual interval in the standard. Retraining is required when the duties change, when there is a change in the permit space operations, or when an evaluation shows the training did not stick, meaning deviations from procedure or gaps in someone's knowledge. Most employers run it annually anyway because it is easier to schedule and easier to evidence than proving no trigger occurred.
